The prevalence of chronic illnesses among children and adolescents has increased significantly worldwide over the past decade, posing a growing coordination challenge for healthcare providers. Hospital pediatric departments (HPDs) play a crucial role in caring for chronically ill children and adolescents in Germany. The 'HPDnet' project analyzes the care of pediatric patients with complex, multicenter care needs, exemplified by (chronic) kidney disease. In Germany, the number of HPDs specializing in pediatric nephrology is relatively low. Additionally, a large number of different healthcare providers and caregivers are involved in the treatment of pediatric patients with (chronic) kidney disease. The medical care therefore requires a high degree of coordination and cooperation between the pediatric patients, their caregivers and the healthcare providers, and is typically characterized by a considerable degree of inefficiency. The interdisciplinary research project aims to investigate how HPDs can be efficiently organized in healthcare networks in order to improve the coordination between different stakeholders and the quality of healthcare for children and adolescents with chronic diseases.

Brief summary in scientific language

The project comprises of four main work packages (WP). Research focus is laid on the analysis of structures, networks, and care processes in pediatric departments of hospitals. Particularly, patient flow characteristics across hospitals and patient outcomes (WP1), organizational factors, proximity, and treatment expertise (WP2), regional patient flow network patterns and relational coordination (WP3) as well as facilitators and barriers of patient-centered care (WP4) are investigated. Work in all packages can be linked to care-determining and to patient-relevant endpoints. In addition, the different perspectives of stakeholders will be considered and evaluated. Based on the results from the different work packages, a process-oriented intervention to improve coordination and cooperation will be designed, thus improving the care of pediatric patients with (chronic) kidney disease.

Interventions, Observational Groups

Arm 1:
WP1: Regional analysis of patient pathways Retrospective analysis of the care pathways of children and adolescents with chronic kidney disease in the Cologne care region across all healthcare providers involved on the basis of secondary care data from the Cologne Competence Network of Practice and Research (CoRe-Net) to identify and analyze the characteristics of patient flows.
Arm 2:
WP2: Analysis of organizational factors, regional care infrastructures and treatment competencies Retrospective analysis of structured quality reports from German hospitals to evaluate pediatric care infrastructures using descriptive and inferential statistical methods, including analysis of pediatric-nephrological expertise and treatment spectrum with final cartographic representation.
Arm 3:
WP3: Analysis of regional networks and relational coordination Primary data collection among pediatric hospital departments nationwide for exploratory identification of existing care networks based on analysis of direction and volume of patient flows. Analysis of relational coordination and organizational cooperation within the regional network structures.
Arm 4:
WP4: Analysis of determinants of patient-centered care (PCC) Qualitative survey of patients and their informal caregivers as well as professional healthcare providers on barriers and facilitators of patient-centered care for patients with chronic kidney disease in pediatric hospital departments. Evaluation of the effect of PCC on the improvement of existing care processes.

Endpoints

Primary outcome:
Direction (proportion of admissions and referrals per department) and volume (relative and absolute number of pediatric-nephrological cases) of patient flows in regional care networks
Secondary outcome:
WP1: Complexity (number of care providers involved), intensity (contacts with a care provider) and dynamics (time periods with extremely low/high intensity and complexity) of patient-provider relationships for the Cologne care region (secondary data: CoRe-Net) WP2: Organizational care indicators of pediatric-nephrological cases (number of cases) and care expertise (number and degree of specialization of pediatric hospital departments) (secondary data: structured quality reports) WP3: Relational coordination and cooperation factors for the care of pediatric-nephrological cases of healthcare providers (survey) WP4: Patient-centered care of nephrology-pediatric patients (interviews)

Exclusion Criteria

- Inpatient department without pediatric specialization (according to department code) and/or care focus on diagnostics and therapy of pediatric kidney diseases (VK 04) - Treatment of <10 cases from study-relevant ICDs per year
